Untuk makluman, Jabatan ini masih membayar Elaun Latihan Amali kepada semua pelajar tajaan yang berkelayakan dan memenuhi syarat sepertimana amalan sediakala dengan syarat seperti di bawah:

1) Memohon secara rasmi kepada JPA melalui pihak universiti;

2) Permohonan tidak melebihi 90 hari (sepanjang pengajian) ; dan

1. Latihan Amali yang diambil bermula pada Sem I 2007/2008 dan seterusnya sahaja yang layak memohon.

2. Maksimum hari yang boleh dituntut untuk Latihan Amali ini adalah 90 hari sahaja.

3. Jarak tempat Latihan Amali mestilah lebih 25km dari tempat pengajian. Jika Latihan Amali dibuat di universiti masing-masing, maka pelajar tidak layak untuk memohon.

4. Permohonan tuntutan hanya boleh dibuat selepas Latihan Amali habis dilaksanakan.

1. Salinan muka hadapan buku akaun bank (disahkan).

2. Surat Pengesahan dan Cop Majikan tempat pelajar membuat Latihan Amali. Pengesahan dalam jumlah hari kedatangan perlu disertakan dengan salinan punch card serta tarikh pengesahan dibuat.

3. Jika sistem punch card tidak digunakan, kemukakan salinan buku log kedatangan atau surat pengesahan setiap bulan yang diperakui oleh majikan.

4. Surat pengesahan universiti (kuliyyah).

5. Borang Tuntutan Elaun Latihan Amali daripada JPA. Boleh ambil kat OSC. Borang sama yang pernah diisi dulu.

Attachment letters.

Gen2welve members who sent their attachment letters, your signed letters are already with me, we may need to meet up to pass you the letters and for me to brief you regarding the procedures asked by Dr Shahrin for you to adhere to. Is it possibble for us to meet up this friday? Head of posting (HOP) please confirm with me your timetable so I can arrange the meeting.

Yep, all of us pretty much realize how close we are to the moment/date/exam of truth. But hey, look at the bright side. If you think all theories and no practical practically make you no fun of a guy to be around, I heard clinical years are just the time for you to shine!

Oh ya, by the way, congratulations to Sulaimi and Razan, our newest lovey dovey couple of ‘the block’! Godbless to them both. Pictures, of course, are coming up later. So hold your breath and relax. Or better yet, go back to your notes. You still have a lot of anatomy and physiology to cover hahaha :D

Never Too Late for Anything…

…as long as you want it.

When people say that there is no end to human desire, I was wondering how would that work. Won’t a person realize his/her limitation; what one can do, what one can’t do, how far would one go to get something, how much would one sacrifice to gain?

Then I realize something. Limits are by our own definition; we set it. If it is your limit, then that’s that. But sometimes it is just us who don’t want to go that extra extent because we are afraid of what the cost might be, and sometimes pure laziness and and spiritless soul make you underrate yourself. When you can obtain more, you stopped at where you are. At some points in life that I haven’t encounter, maybe that’s the best. But other than that, why wouldn’t we push our limit?

It is even sad to think that we didn’t push ourselves not because we can’t, but because we don’t want to.

That’s what I get from the latest issue of Milenia Muslim, March 2010 edition; an article written by Prof Dr Muhd Kamil Ibrahim, the writer of “Travelog Haji – Mengubah Sempadan Iman”. He has an ambition, and he pushed through all the hurdles and difficulties to get through. Along the way, he found out that he lacked in a very vital part of being a human; even more so, a Moslem. He lacked the basic foundation of ‘ebaadah and most of other part. Yes, he prayed, but he himself realized that what he did was not enough. He didn’t even know how to recite the holy book. In essence, he’s a professor in his field, but at the basic level of being a moslem, he was a failure.

It is interesting to see in the article that his pursue of his ambition and career got him closer to the Creator. He then pushed himself through and finally get to know more about Islam, and being able to practice it in a more comprehensive, complete way gave him satisfaction.

What I wrote won’t do his article justice, but that’s my take. There’s a lot of thing that we can actually learn from him; but the most prominent one is when you want something, you set your heart to it, fall in love with it, and this will drive you to achieve what you set in the first place.

Go read the article. Not to do advertisement or anything, but Milenia Muslim can be found anywhere. If you happen to see one, go get it.

A(H1N1) flu: Updates on 10 FAQs — Dr David KL Quek

AUG 6 — 1) Can we distinguish between regular and H1N1 flu, without a lab test?

No, the flu is the flu, but there are variations in presentation. Some symptoms such as cough, runny nose, fever, body aches, fatigue, vomiting, diarrhoea occur more or less in every flu patient, but may present differently by different people. Some infected people have very mild symptoms, some in between, and a small minority, probably less than 10 per cent, have severe features including the dangerous pneumonia.

However, from sentinel testing and surveillance by the Ministry of Health the last few weeks have shown that almost 95 per cent of all flu-like illness are now caused by the H1N1 virus. Earlier some months ago, seasonal flu variants caused by the B and other A virus were the main causes, the bug causing most flu these few days is the A(H1N1). This appears to be the case also in neighbouring countries, meaning that the new virus is causing more havoc and symptomatic illness than previous types of flu (which are still in the community).

Because almost every flu-like illness (influenza-like illness or ILI) is due to H1N1, the MOH is now recommending that no testing to confirm this H1N1 will now be offered.

Treat as if this is H1N1 for ILI — symptom relief for mild symptoms (paracetamol, hydration, cough medicines, etc) and self-quarantine, social distancing, be alert for complications.

Most (70 per cent) do not need any anti-viral medications such as Tamiflu or Relenza. Only severe cases need to be referred to hospital for further treatment.

2) How should doctors decide if a person be given further specific treatment for H1N1?

If after 2-3 days, fever and cough symptoms do not improve, a recheck with the doctor is recommended, especially if there are features of difficulty breathing, severe weakness and giddiness, or, if the following risk factors are present:

1. obesity (fatter patients seem to have poorer outcome and more complications)

2. those with underlying diabetes, heart disease

3. those with asthma, or chronic lung disease

4. pregnant women

5. those with reduced immunity, cancer patients, etc

6. those with obvious pneumonia features

3) Many anxious people with flu-like symptoms want to be tested or treated for suspected H1N1, but are kept waiting or sent home, without being tested. Is this practice right?

There is no right or wrong practice as this outbreak is extensive and is stretching our resources to the limit. This is also the case not just here in Malaysia, but also elsewhere around the entire world!

The recommendation is now not to spend too much time and effort trying to get tested at designated hospitals or clinics — there is probably no need to do so. I have been informed that as many as 1,000 patients queue anxiously at Sungai Buloh Hospital for testing, due to fear of the H1N1 flu.

So the message must be made clear: Most flu illness do not require confirmatory testing, and are mild and self-limiting. More than 90 per cent will get better on their own, with symptomatic treatment — just watch out for possible complications, and risk factors as mentioned above.

Our resources are limited especially for testing. This is not just for Malaysia, but globally as well. The global demand for test kits and reagents for the H1N1 (PCR) is overextended and are rationed due to this extreme demand.

Some 200 million test kits have been deployed worldwide, but this supply is critically short because of excessive demand, so most countries have to ration testing to confirm only the worst cases, so as to monitor the pandemic better.

4) Are doctors confused as to what to do in this outbreak, especially when they do not have ready access to confirmatory lab tests?

Not really. Earlier on there was some confusion as to what to do next and who to test or who to refer for further testing and admission. Now the rules are clearer.

There is no need to do any testing to confirm the H1N1 virus for any ILI — just assume that this is the case in the majority of cases. Treat symptomatically when symptoms are mild, reassure the patients and ensure that these infected patients practice good personal hygiene, impose self-quarantine and social distancing, wear masks if their coughing or sneezing become troublesome, and keep a watchful eye on whether the infection is getting better or worse.

If there is difficulty breathing and gross weakness, then patients should quickly present themselves for admission. Understandably this phase of worsening is not always clear or easily understood by everyone... But there is not much more that we can do — otherwise we will be admitting too many patients and this will totally overwhelm our health services.

But prudent caution would help to determine which seriously ill patients need more attention and more intensive care. Unfortunately however, there will be that odd patient who will progress unusually quickly and collapse even before anything can be planned — hopefully these will be few and far between.

A more important note is that all doctors and nursing personnel should be very aware that they too have to take precautions, and employ barrier contact practices, if there are patients with cough and cold during this period of H1N1 outbreak, which is expected to last a year or two. Carelessness can result in the physician or nurse or nurse-aide becoming infected!

5) Are there sufficient guidelines from the Ministry of Health to address this situation?

I think there are sufficient guidelines from the MOH. Although some politicians have blamed the MOH and the minister for being inept at handling this pandemic — in truth this is not the case.

It is useful to remember that this is an entirely new or novel virus, which no one previously had encountered before — thus its infectivity and contagiousness is quite high and almost no one is immune to this virus.

Perhaps, there will come a time when all the resources from both public and private sectors can be put to more efficient use. Some logistic problems will invariably occur, because human beings differ in their capacity to understand or follow directives, whatever the source or authority.

Also patient demands have been extraordinarily high and at times very difficult to meet — every patient necessarily feels that his flu is potentially the worst possible type and therefore requires the most stringent measures and testing...

Doctors are also unsure as to the seriousness or severity of this new ailment — and we are only now beginning to understand this better — so our less than reassuring style when encountering this new H1N1 flu is sometimes detected by an equally anxious patient and/or their relatives.

But there is only so much that we can do under such a pressure cooker of an outbreak which is spreading like wildfire! But nevertheless we should not panic, and remember that most (more than 90 per cent) of infected people will recover with very little after-effects. Possibly only one in 10 patients develop more serious problems which necessitate hospitalisation.

6) Is limiting H1N1 testing only to those who have been admitted to hospital justifiable?

I have explained the worldwide shortage of such testing kits and reagents. Also it is near impossible to test everyone, the world over. Besides, knowing now that almost all the flu-like illness in the country is due to H1N1 makes it a moot point to want to test for this, especially when most are mild.

The rationale for testing only those who need hospitalisation is to ensure that we are dealing with the true virus, and also help to isolate possible changes or mutations to this viral strain. The MOH is also constantly doing sentinel surveillance (random spot-testing at various sites around the country to determine more accurately the various virus types and spread that are causing ILI).

7) Are we short of anti-virul drugs (Tamiflu, Relenza)? Should I take Tamiflu?

These antiviral drugs were available to most doctors during the earlier scare of the bird flu virus, but now are severely restricted, although some orders are still entertained from individual doctors, clinics or hospitals. Remember that these have been block-booked by more than 167 countries which have been shown to have been penetrated by the H1N1 flu bug.

Our MOH has actually stockpiled some two million doses of the Tamiflu or its generic form. In the last inter-ministerial pandemic influenza task force meeting, this stockpile will be bumped up to 5.5 million doses to cover some possible 20 per cent of the population.

Right now there is no shortage in the country. It is just that it is not readily available on demand for anyone just yet. The MOH is still of the opinion that this antiviral drug be used prudently and would like to register every patient given this drug.

The private sector on the other hand would like to have a looser control over the use of this drug — but we acknowledge that we should be meticulously prudent in its use. There is a genuine fear that resistant strains to this drug may develop with indiscriminate and unnecessary use — then we will all be in trouble with a drug-resistant H1N1 virus run amok!

Drug-resistant strains have been detected in Mexico, border-towns in the US, Vietnam, Britain, Australia even. So we have to be vigilant and closely monitor the situation. Right now, the very limited usage of Tamiflu gives us good reason to be optimistic.

However, because of some unusual patterns of seemingly well people dying or having very critical infections, some people and doctors are wondering if these new strains have already reached our shores... or have we been too late in instituting proper treatment...?

The rising number of deaths to 14 now is quite worrisome, but our health authorities are watching this development very closely and are also checking the virus strain to see if this has mutated. We can only hope that this is not the case, for now.

8) What are some of the problems faced by doctors in dealing with the H1N1 problem?

It would be good if every medical practitioner keeps a close tab on the H1N1 pandemic, and remain fully aware of the developments and changes, which are evolving daily. Every doctor has to be learning on the trot, so to speak, to keep up with the progress of this outbreak and its management, so that we can serve our patients better.

Logging in to the Internet regularly for more updated information will certainly help, instead of lamenting that not enough is being disseminated via the media thus far... Every doctor has to be more proactive and practice more responsible and cautious medicine during this trying period which is expected to run into at least one to two years. Importantly, look out for lung complications, and the above stated higher risk profiles, and refer these patients quickly for further care.

Easier access to antiviral drugs and their responsible use and monitoring would help allay public fears of delay in treatment, but this should be tempered with care and not over-exuberance to dish out to one and all, the precious antiviral drug, just for prevention — this may be a very bad move which can inadvertently create a worse outcome of drug-resistant bugs.

However, in the light of the very quick deterioration of some young patients who have died, it might be prudent to use antiviral treatment earlier and more aggressively.

We look forward to the specific H1N1 vaccine, when it does come our way, probably towards the end of the year. In the meantime, encouraging those in the front-line, heart or lung patients and frequent travellers to have the seasonal flu vaccination is a useful adjunct to help stem the usual problems from other flu types.

9) Are we doing everything that should or needs to be done?

Yes, if you check what other nations are doing, we are doing relatively well. We are not overstating the dangers and we have been quite transparent on the possibilities of this pandemic. Earlier, many agencies and even the public and doctors have accused us of exaggerating the pandemic, and our response was dismissed as being too much, even over the top! Unfortunately, it was only when some deaths occur that many are now decrying that we have done too little!

Also if you are quite honest about it, just compare with the countries globally, and you will notice that no one health or government authority has got this right, spot on.

We are all learning about this novel flu pandemic, and each country's response is coloured by its past experiences. In Hong Kong, China, Vietnam, Singapore and Malaysia we have had the SARS outbreak, so we are necessarily more paranoid! Also here the experience is that flu does not usually cause death in our community, unlike the west where seasonal flu kills some hundreds of thousands every year!

So the fear factor for this H1N1 flu is not nearly as great in the West, although it is slowly sinking in that its contagiousness and infectivity is far greater, and fears of its reassortment to a more virulent mutant form are growing, into the so-called second and/or third wave of this pandemic, but we will not know until a year or so down the line.

10) Is the public in general doing enough to help in controlling the outbreak?

I think the public is now reasonably well-informed as to this H1N1 pandemic. Perhaps, they are too well-informed, that they have a fearful approach to this virus. But the proper thing is not too over-react and to panic, although I know this does sound easier said than done.

It is almost a certainty that this flu will spread within the community — in schools, universities, academies, factories, work places, offices, etc. WHO has projected that possibly some 20-30 per cent of the population worldwide will become infected by this novel flu bug, after studying various models of spread of past infections — the huge and very rapid spread worldwide is mainly due to air travel. While older flu pandemics took six months to extend to so many countries, this H1N1 flu did so in less than six weeks!

In the worst-case scenarios of course, this outbreak will be alarming — hospitalisations may be required for 100,000 up to 500,000 Malaysians, with perhaps as many as 5,000 to 27,000 infected patients (depending on the case fatality rate or either 0.1 to 0.5 per cent) succumbing to this illness.

But because we have been monitoring closely and containing the outbreak thus far, with heightened awareness and greater social responsibility, it is possible to ameliorate the infectivity, spread and fatality that will unfortunately accompany this pandemic... Just how successful we will be in limiting these adverse outcomes remains to be seen, but we can be hopeful.

How can the public help? First learn and acquire good personal hygiene. If sick, please be responsible and stay at home, even in your own room where possible, wear a face mask (a cheap three-ply surgical mask will do, because large droplet spread is the main danger). Do not go out, practice what is now known as social distancing (about three metres from anyone), and be socially responsible, don't go to public places and infect others — for young people this would be hard, but absolutely necessary — the spread is most rampant in this age group between 16 and 25 years.

When the illness does not go away after a few days or when you are deteriorating, get to the nearest hospital. Most importantly, be very aware and responsible!

Finally, keep abreast of all new developments, because these are evolving all the time. With keen awareness, prudent care, early detection and social responsibility, correct and prompt use of antiviral and other support medical care, and later mass specific vaccination, we can overcome this novel H1N1 flu! But it will take time, patience, public cooperation, much concerted effort and consume great resources.

Let’s play a ‘What If…’ game.

What if a news struck you that your son is having a perimembranous ventricular septal defect (jantung berlubang), and you need RM18000 to cover the operation to do something about that, exclusive of other needs to cater the medical attention service charged by IJN, estimated to total up to RM28000?

Well, some of us who are well-to-do would give no hoot about forking out that amount; as long as our child get to lead a healthy life.

But what if you are not well-to-do, and in fact, calling this kind of house your home?

Bedroom, washroom, toilet, kitchen, all you can think of in a common space. Some of us might even have, in their mind, that to have three person in such a confined space is unthinkable.

What if your source of income is solely the odd jobs you do, looking for junk that, only, might be able to cater your daily need?

That is exactly what you can found in Kuantan; heck, i don’t think this is the only case you can found in Malaysia. But yeah, to even think that there is such case of extreme poverty in the middle of well-to-do residential in the area of Taman Zurina, Jalan Seri Teruntum 96, Kuantan; the fact is downright astounding.

This is the son in need of help. The surgery is scheduled on this August.

Alright, guys! This will be the new layout; the last one was rather too black and gothic looking, like it was some funeral blog, if there are any thing of that kind. this one is simple, and more straight-to-the-point kind of layout; but please don’t misjudge this blog for all-work-no-play; many things will, from here on, be updated in here. Tenteikuura will still be our first information dissemination tool; but this one will do what tenteikuura can’t. Got the gist? :D

Somebody’s getting married!

Yeap, our friend, InsyaAllah, is going to get married this June.

Most of us, his batch members didn’t see this coming. Fast. I mean, all of us are in a way or another ‘have plans’ right? Those who are single will want to end it; those who aren’t single might want to go for number 2 (bwahahaha!!!)

He will be the number two in my class to be a husband. The first place was taken even before we started our session here in IIUM; during matriculation to be exact; both the husband and wife are my classmate. Both blessed with a cute baby Iffah :)

Just a quick update about this and that…

1. remember, we are all going to go visit HTAA this week; by groups in three successive days. make the most of it people! it is going to be informative, and fun.

2. dr pakeer’s practical session; remember what he wanted us to have? yeap, that. exactly that. he wants us all to submit drawings we’ve made for whatever we have observed during the many sessions of parasitology practical we have had. it was very thoughtful of him for wanted us to submit it right before our OSPE session. so be sure to make the most out of it :) oh ya, no matter how dirty or terrible looking our drawing is, don’t worry, just submit whatever you have in your hand lah. i’m sure there will be no such “your drawing makes me dizzy, so you fail your exam” kind of thing hehehe :D

3. we have so many things to read. read through, get acquainted with all the subjects, reread whenever necessary; all will be alright.

4. we should have a solat hajat together, don’t you think? so i think the batch committee will try to organize one; where all brothers and sisters will get together and pray to the power we are depending onto that we will do well :)

KLANG: Teachers and students at a secondary school in Kapar had the scare of their lives when an armed man stormed into the school and stabbed a student.

Klang OCPD Asst Comm Mohamad Mat Yusop said the incident occurred a few minutes after the morning session ended at 1pm.

He said the man, who was wearing a crash helmet, entered the school compound with a knife in his hand.

A female teacher tried to stop him but he quickly walked up to a group of students and stabbed one of them in the arm and thigh, he added.

The assailant then walked out of the school compound and slashed three more students who were waiting to go home.

The three students, however, suffered only minor injuries.

All the victims were aged between 14 and 15.

The four students were rushed to the Tengku Ampuan Rahimah Hospital here for treatment.

The student who was stabbed was admitted while the others were given outpatient treatment.

ACP Mohamad said police believed it was a revenge attack following a gang fight between two groups of students on Tuesday.

The assailant, believed to be a former student, is still at large.

Students' task :

A mail hit my box yesterday. Sorry sis Elviza, for not responding as as quickly as possible. Been having some joyous moment around Sg Buloh, Shah Alam, Klang…now I am feeling bad; for not prioritizing what’s more important…

I believe all of us want to help in a way or another; I mean, we are having our brothers killed there. Here’s one way. Sis Elviza has been with Mercy Malaysia for sometimes already, and they have Palestine Relief Fund set up on 30th December 2008.

The air strike and ground offensive on Gaza - as reported by AlJazeera - have killed more than 700 Palestinians, 219 of which are children. More than 3000 have been wounded.

Anyway, we are getting close; in fact we are counting down the days, with our final block exam eh? Well, nothing much I can really say out loud. Luck, well, not going to work very much when loads of reading is what you need to nail this one nice and good. So, guys, anything you haven’t finish with, get down to it. NOW! 10 days, not that long time, if you fill it with all the procrastination, that is. But come on, wonders can be made within 10 days :D

Man, I can’t sleep…

You got sleep problem? Well, I do; especially when you have things bugging you (eg. …exams?).

I daresay that everyone of us have had this; you know, tossing around on bed, awoken in the middle of nights, feeling some kind of fatigue upon wake up, blogging at hours you should be dreaming…well, you know what I mean.

Actually, you can help it out by some simple steps; unless you are suffering from serious sleep disorders; that one, man, go see doctor.

Our sleep is affected by various factors; mostly body clock which is very photosensitive. What we eat also apparently giving out effects. Now, if you think you have occasional trouble going to sleep, do this. Drink milk. Yup, you heard it right. No bluff, pranks, whatever. And seriously, man, you don’t look less masculine drinking milk. This is biochemistry we are talking about, mister.

An essential amino acid (requires intake through diet), namely tryptophan, is a brain calming material. Our body converts tryptophan into melatonin and serotonin; which are thought as sleep inducer. Combined with some carbohydrate in the diet, say, bread, you’re good to go. Carbohydrate and calcium help in the usage of the tryptophan.

Milk alone of course won’t do the trick. But it will definitely help you get a better night sleep. You need one; or else, your performance in the following day will be impaired.

Microsoft Examines Causes of ‘Cyberchondria’

If that headache plaguing you this morning led you first to a Web search and then to the conclusion that you must have a brain tumor, you may instead be suffering from cyberchondria.

On Monday, Microsoft researchers published the results of a study of health-related Web searches on popular search engines as well as a survey of the company’s employees.

The study suggests that self-diagnosis by search engine frequently leads Web searchers to conclude the worst about what ails them.

The researchers said they had undertaken the study as part of an effort to add features to Microsoft’s search service that could make it more of an adviser and less of a blind information retrieval tool.

Although the term “cyberchondria” emerged in 2000 to refer to the practice of leaping to dire conclusions while researching health matters online, the Microsoft study is the first systematic look at the anxieties of people doing searches related to health care, Eric Horvitz said.

Mr. Horvitz, an artificial intelligence researcher at Microsoft Research, said many people treated search engines as if they could answer questions like a human expert.

“People tend to look at just the first couple results,” Mr. Horvitz said. “If they find ‘brain tumor’ or ‘A.L.S.,’ that’s their launching point.”

Mr. Horvitz is a computer scientist and has a medical degree, and his fellow investigator, Ryen W. White, is a specialist in information retrieval technology.

They found that Web searches for things like headache and chest pain were just as likely or more likely to lead people to pages describing serious conditions as benign ones, even though the serious illnesses are much more rare.

For example, there were just as many results that linked headaches with brain tumors as with caffeine withdrawal, although the chance of having a brain tumor is infinitesimally small.

The researchers said they had not intended their work to send the message that people should ignore symptoms. But their examination of search records indicated that researching particular symptoms often led quickly to anxiousness.

They found that roughly 2 percent of all Web queries were health-related, and about 250,000 users, or about a quarter of the sample, engaged in a least one medical search during the study.

About a third of the subjects “escalated” their follow-up searches to explore serious illnesses, the researchers said.

Of the more than 500 Microsoft employees who answered a survey on their medical search habits, more than half said that online medical queries related to a serious illness had interrupted their day-to-day activities at least once.

Mr. Horvitz said that in addition to his interest in creating a Web search tool that would give more reliable answers, the research was driven by clear memories from his medical school education of what was often referred to as “second-year syndrome” or “medical schoolitis.”

He said he remembered “sitting on a cold seat with my legs dangling off the examination table,” convinced that he was suffering from a rare and incurable skin disease.

While the doctor was out of the room, Mr. Horvitz said, he took a look at his medical chart and saw that the doctor’s notes read, “Eric is in medical school, and he has been reading a lot.”

The researchers said that Web searchers’ propensity to jump to awful conclusions was basic human behavior that has been noted by research scientists for decades.

In 1974, the psychologists Amos Tversky and Daniel Kahneman wrote a seminal paper about decisions that are based on beliefs about the likelihood of uncertain events, like the outcome of an election or the future value of the dollar.

They said that people usually employ common sense rules to aid in decisions. The rules can be quite useful, but they also frequently lead to systematic errors in judgment.

The Microsoft researchers noted that reliance on the rankings of Web search results contributes a similar bias to the judgments people make about illness.

At the same time, Mr. Horvitz said he believed that the Web would evolve to offer more reliable information.

In the 1990s, Microsoft researchers built a health advisory system for pregnancy and child care. Mr. Horvitz said that in the future it would be possible to create search engines that were able to detect medical queries and offer advice that did not automatically make Web searchers fear the worst.

Physicians have suffered a significant reduction in the prestige and respect that they once enjoyed. There are many reasons for this: the commercialization of the practice of medicine; the surrender of control of the physician-patient relationship to insurance companies, health maintenance organizations, and other business entities; and the increasingly high cost of delivering medical care. Even when most
of the increased costs are driven by drug, hospital, laboratory, and radiological fees, these are perceived by many patients as "doctor bills." To maintain viable practices, physicians must increase their patient volume and spend less time with each, which contributes to the diminution of physicians in the eyes of the public and the changed image of our calling as a business rather than a profession. With an increasingly cynical, testy, and confrontational populace exemplified by the behavior on both sides of the political spectrum during election campaigns, we physicians find ourselves "less loved," and, in some cases, "dissed" (using street language), and we don't like it.

Most individuals enter the profession of medicine because of a sincere desire to alleviate suffering in their fellow man. If, however, you "scratch below the surface," you will find that many of us also want to be loved and respected for the dedication that we exhibit and the valuable service we render. This is not unique to our profession, but physicians, like other medicine men and women since time immemorial, have usually occupied positions of respect in the tribe or society in which they have lived. Today, we feel ourselves slipping from that pedestal.

Today, anyone with a computer can go online and find multiple discussions of any symptoms, disease, or other disorder plus recommendations, not always valid, for correcting the medical problems. So, do we need the physician anymore? More people are seeking medical care from physicians than ever before, and it is not just because they haven't figured out a way to perform surgery over the Internet.

For those of us who studied science in high school, worked hard to get into a good college where we diligently studied to get accepted into medical school, where we studied diligently to obtain a residency, where we worked hard to join a successful practice, where we worked hard to pay the bills, oblivious to the changing world around us, we have had little opportunity to evaluate and contemplate these issues. It is not until you retire, as I have, that you have the opportunity to consider these issues and determine the solutions, if any, but by this time, even if you still care (and some don't) or you can still think (and some can't), few will listen to someone who may be considered to be out of touch.

If any of what I have said rings true, let me offer you some advice that may assist in negotiating the bumps in the road of your career.

GIVING ADVICE

First, be chary of giving advice; wise people don't need it, and fools won't heed it. Sometimes it is worth what you pay for it. As physicians, however, our advice is often sought, and we need guidelines on how to provide it. So, here is some free advice. Remembering it may be worth what you pay for it.

Consider that you won't always have the answer to a patient's question, so don't be afraid to admit it. Owing to the amount of medical information available to laymen, patients sometimes already know the answers to the questions they ask; they may be testing you and can recognize a false answer. Also, don't be too certain that what you were taught as fact is so. When I graduated from medical school in 1960, I thought I knew all the answers to all the medical questions. It all went downhill after graduation, and it was not long before I discovered that I didn't have all the answers—that putting an X in the right box on a multiple-choice examination was far different from putting my finger on the right spot in the examination of a patient. Physicians have an obligation to learn as much as possible about what is considered current, but they also have an obligation not to accept something until it has been studied properly and proven.

Physicians should be as scientific as possible, which means having an inquiring mind open to new facts and not being reluctant to question old ones. As Alexander Pope said, "Be not the first by whom the new are tried, nor yet the last to lay the old aside."

ETHICS

Physicians should embody the highest standards of ethics: honesty, integrity, morality, and fidelity. These virtues are regarded as old fashioned by many who subscribe to postmodern, secular thinking, and they are indeed old fashioned, if by that one means they have been around for a long time. These principles have survived because it has been learned, sometimes through bitter experience by failing to apply them, that these standards are the best way to live. Fashionable today are situational ethics, the concept that the truth changes with the circumstances. Nothing is certain; people don't do something without an ulterior motive; virtue is no longer its own reward. More likely the driving force is, "If it feels good, do it!" There may be professions that can adopt this thinking and survive (although I doubt it), but medicine is not one of them. To the extent that we have adopted this thinking, it can be directly related to the degree to which our respect has fallen.

It is crucial that a physician always be truthful, and truth does not change with circumstance. This doesn't mean that what is said is always 100% accurate or correct; I used to tell the residents that half of what I was teaching them was wrong, I just didn't know which half it was. One should always relate what he or she believes is true. This may be more difficult than one might think. I've had people say to me, "If Granddad has cancer, don't tell him. It would just kill him." My response has always been, "If Granddad has cancer, he has a right to know, and he didn't get to be Granddad by avoiding the truth. While I will be sensitive to the situation, if he asks me, I most certainly will tell him the truth." The net effect of this is to increase your veracity in the eyes of the family members, because if you would lie to Granddad, you would probably lie to them. Also, I was always too busy to keep different stories straight.

I also told the residents and students that in my experience, there are 3 things that have diminished or ruined careers: drinking too much alcohol, financial improprieties, and sexual involvement with the patients or those with whom you work. These 3 things are frequently related. This is not only a moral issue, it is good business to avoid these traps.

HUMILITY

Humility is a virtue that not many physicians possess. This is understandable when one considers the life-and-death decisions physicians are sometimes called on to make and the adulation we sometimes receive from patients and their families. We must, however, always guard against becoming arrogant. Not all of the patients we treat have good outcomes. Many patients will either die or get well in spite of what the physician does. Voltaire said prior to 1778, "The art of medicine consists in amusing the patient while nature cures the disease." Or, as Ambrose Paré, the father of French surgery stated before 1590, "Physicians treat, God heals."

TALKING TO PATIENTS

I'm always amazed when I hear a physician, often young but not always, say to a patient with cancer or some other potentially fatal disease, "Madam, you have 1 chance in 3 of living 2 more years if you follow my recommendations. " Or, worse, "Only 59.5% of patients with your disease will live 5 years." This is applying a scientific precision to a situation that cannot be that precise.

Statistics are fine for writing papers or evaluating a given treatment, assuming valid controls are present, but a single patient is not a statistic, and you should not make the patient into one. If 90% of a particular group of patients will be dead in 5 years according to statistical analysis, it follows that 10% will be alive in 5 years. To which group does the patient you are addressing at the moment belong? Granted, patients have a right to know generally their chances of survival in order to make plans, but as physicians, we don't always know what these chances are for a specific patient. I've seen patients who I thought did not have much chance of surviving who went on to respond well to treatment and live many years. Conversely, I've seen patients who seemed as though they would do very well deteriorate after standard treatment and die. It is not easy to predict outcomes.

Norman Cousins, former editor of the Saturday Review of Literature, was once diagnosed with a terminal illness. Rather than seek palliative therapy in a hospital with an undetermined outcome, he chose to enter a hotel, consume 25 000 U of vitamin C daily, and watch old Marx Brothers movies. He believed he laughed himself well. This episode stimulated him to write a book titled Anatomy of an Illness: Reflections on Healing and Regeneration1 (it is still in print, and it is recommended reading for all physicians). On the dust jacket, Cousins wrote these words, "It all began, I said, when I decided that some experts don't really know enough to make a pronouncement of doom on a human being. And I said I hoped they would be careful about what they said to others; they might be believed and that could be the beginning of the end."1 His point, of course, was that physicians should not destroy the hope of patients. In his later years, Cousins served on the faculty of the University of California, Los Angeles, Medical School, reminding the students of these lessons.

Be careful what you tell your patients; they are fragile beings, wonderfully constructed, but words can damage them just like a knife, and they place great trust in you. No person has a right to destroy the hope of another person.

Use language that patients understand. This may be more difficult than you might imagine. I once undertook an unpublished study on informed consent. I would carefully explain to my preoperative patients what type of surgery was to be performed. I would then ask them to write on the consent form what they understood the procedure to be. Fully half could not name the procedure I had just explained to them. Patients will rarely admit that they don't understand, and they are often more frightened than they will admit. When they enter a physician's office, or are seriously ill, many are not very perceptive or receptive. Small wonder then, that some malpractice cases have been brought by patients who say they were not informed by the surgeon of what he or she was going to do in spite of a consent form in the medical chart, signed by that patient, that clearly outlined the procedure with all the possible risks and complications. One moral of this story is that keeping accurate, complete, and clearly written records is a must. If your handwriting is illegible, as with most physicians, have it typed. This is good patient care as well as preventive legal care.

Offer a compassionate, caring attitude to the patient. Many patients today complain about the care they receive or fail to receive. In fact, from a strictly scientific view, this "delivery of medical attention" today is perhaps the finest ever provided in the history of the world. However, aside from its significant cost, it is frequently perceived by the patient as not being delivered with care. The aloof, scientifically oriented, detached physician may be totally correct in his or her diagnosis and treatment but end up with a patient cured but dissatisfied that they did not receive the proper care.

HUMOR

I believe the physician should possess and exhibit a sense of humor. As Cousins showed,1 laughter is great medicine. There is some evidence2-3 that endorphin levels rise when one laughs and are lowered when one is dour or depressed. Without being flippant, you should always try to exhibit a positive attitude toward patients. I treated mostly head and neck cancer during my career, and when a patient was given this diagnosis, they were usually appropriately scared. Some would ask, "What are my chances? Am I going to die?" My response was always, "Yes." After the shock of this fairly brusque reply had passed, I would add, "Everyone must die sometime, but I am here to see that this doesn't happen to you on my watch." I would then outline what treatment plan the tumor board had recommended, and go on,

“This is what we are going to do for you. And here is what you are going to do for me. You are going to quit smoking and quit drinking. If you are unwilling to do this, you will need to find another doctor, because otherwise, you will just be wasting your money and my time. People who develop these cancers, usually because of smoking and/or drinking too much, even when cured by the therapy we recommend, if they continue to smoke or drink have a 50:50 chance of developing a new cancer within 2 years.”

All of my patients did not stop smoking or drinking, and I never turned any away, but many did quit, and all were surprised that I felt so strongly about these vices. Some said that no physician had ever "told" them to quit before. I found this hard to believe and most curious until I analyzed exactly what they were saying, "No physician told them to quit." The physicians would recommend or advise them to quit but did not order them to do so. Perhaps it was my military background, but it was not difficult for me to give an order. You can disregard a recommendation or advice, but you must disobey an order. Some of my colleagues have argued that we don't have the right to order patients in this fashion. Really? We write orders all the time for such picayune things as when a hospitalized patient can get up to go to the bathroom or not, what kind of diet they can have, and what drugs they should take. Why shouldn't we order them to cease something that is so deleterious to their health?

Don't take yourself too seriously. We physicians, working long hours, dealing with disease, death, and dying (or worse, growing paperwork), sometimes put on a long face and walk around like Digby O'Dell, the friendly undertaker. When things go wrong, as they sometimes will, try to be a little detached and find the humor in the situation. I used to tell myself in such circumstances that someday I would look back at this and laugh. At that moment, however, it didn't seem very funny. Being cheerful at such times is not always easy to do. Allegedly, Winston Churchill's wife, trying to console him after he had led Great Britain during World War II as prime minister only to be voted out of office once the war was over, said, "Don't worry, Winston, this may well be a blessing in disguise." Harrumphed Churchill in reply, "Right now, it is very effectively disguised." Keep smiling; it will confound your detractors. It is, of course, important to always project appropriate gravitas in interactions with seriously ill or dying patients.

One of my pet peeves is the physician who talks down to a patient with a very loud voice. Although a loud voice may be indicated with someone who is partially deaf, in most cases, it is not. It is as though the physician feels that the louder the voice, the truer the words will be, and the more the patient will comprehend. Talk in a normal tone of voice, and treat the patient with respect. You do not have the right to call patients, especially older persons, by their first names unless you also wish to be called by your first name. Elderly people, of whom there are more and more each day and of whom I am one, deserve the respect of you young "whippersnappers. "

PRIORITIES

Where you place your values and priorities tells a great deal about a person. I was told early in my career to first be a husband, then be a father, then be a physician, and then be a specialist. It is good advice, and good priorities, that I tried to follow. Medicine is a very seductive mistress. It is very easy to get caught up in doing good and forget about other important things in life such as family, friends, and enjoying pursuits outside of medicine.

It is also important that one take control of one's life and practice. Understand early on that there will never be enough time in the day to see all the patients who would like to see you, especially if you are a devoted physician. You must be efficient, be on time, and try to adhere to a clinic schedule; do not allow yourself to be lured into a trap of seeing more patients in a given time period than can reasonably be managed. You will "burn out," and the patient will not be happy. Try not to keep the patients waiting. It would probably come as a revelation to some physicians that other people's time is just as valuable as their own. I have always said (and practiced) that if I had an appointment, I would not wait an hour to see a physician unless I was unconscious.

In short, exhibit integrity, honesty, and fidelity. Care for and about your patients, maintain good humor, and don't take yourself too seriously. Speak quietly and with respect in language the patients can understand, and never destroy their hope. Concern yourself with the whole patient. If we physicians do this, we won't have to worry about our place in society; it will be there.

Ms FH, a 32-years old Malay lady represented herself to 'Klinik Warga Sihat' for a regular medical checkup. She was anxious about a few spots below her right eye which has been present for more than one year.

Students' tasks.
1. Are there any terms that you don't understand?
2. Identify the chief complaint.
3.Discuss on all possible causes of the patient's condition and come up with several important questions pertaining to the patient's condition.
4.Build a hypothesis and discuss the underlying mechanism involved.
5.Identify your learning needs.

Guys, here you go. The first trigger for our very first PBL.

Session 1

Part 1 (10 Minutes)

Chief Complaint.
Mr MM a 75 years old man who lives alone was brought to the clinic by his neighbour with complaint of lethargy after not coming out of his house for the last 5 days.

Student's Tasks
You should discuss on the following:

1. Identify the chief complaint;
2. List the information about Mr MM;
3. The term which you may not understand;
4. The possible causes of his chief complaint;
5. Further questions that you need to ask.

Amendment on the first lab handout

Perhaps most of you guys has already change what needed to be changed. Just to make sure that everyone got it, on the page 39 (the second page), please change 'O.D' to 'absorbance'. Whatever 'O.D' means, don't bother. Just focus on the protein electrophoresis, especially on the general idea; and of course the clinical importance. Remember the assignments that we should be prepared with for the whole 2 part of experiments?

1. Describe various factors that influence rate of migration of particles in electrophoresis;

2. Describe applications of electrophoresis in medicine;

3. Describe pattern of serum protein electrophoresis in, a) Multiple myeloma; b) Nephrotic syndrome, and; c) acute and chronic inflammation. The hint, observe which protein fraction elevated or decreased in conditions above;

4. Calculate absorbance of albumin and absorbance of total globulin fraction. Also, find out about normal A to G ratio and what causes low ratio.
